Do my vaccination records (or my daughter’s records) have to go to the HPV Register?
Information will only be provided to the Register with
the consent of the individual or their guardian. If you do
not wish these details to be sent to the Register you should
indicate this at the time of the vaccination. You can ask
for your information to be removed at any time by writing to
the Register, or by completing the
Opt Off
