How do I lodge HPV Vaccination notifications?

Ensure you record a GP Medicare Provider number against each notification. We cannot accept notifications where you have recorded your ACIR practice provider number.

You also need to ensure all fields are completed.  Make sure you have included:

  • GP Medicare Provider Number
  • Patient full Medicare Number including the reference number
  • Patient First Name
  • Patient Surname
  • Patient Date of Birth
  • Patient Street number & Name
  • Patient Suburb
  • Patient Postcode
  • Patient State
  • Vaccination Date
  • Dose Number

If your forms are hand written please make sure all details are very clear and legible. It is preferable to use one of the approved AGPN templates.
