What happens if I have not used any of the approved AGPN templates, state health form or my medical software to record the notifications?

Notifications that are not on one of the approved template, forms or reports from your medical software are being processed by Register staff, but they take a little longer. You should phone the HPV Register on 1800 478 734 (1800 HPV REG) to discuss what format they are held in and how best to submit them.
