General practitioners in all states except Qld, WA and NT

There are three main ways to notify the HPV Register. Register staff are available to discuss the most suitable option for your practice - phone 1800 478 734 (1800 HPV REG).

Please note that HPV dose records are not automatically extracted and transmitted electronically from your practice management software and that the HPV Register is not part of the Australian Immunisation Register (AIR).

  1. Complete the HPV Register notification form which may be enclosed with the vaccine or click here to download a copy. Post the completed form to Reply Paid 725, Sunshine, VIC 3020, or Fax to (03) 8360 8699.
  2. Practice Management Software. Gardasil reports can be extracted and printed, from most popular practice management systems, and instructions can be accessed from the links below: Post the printed report to Reply Paid 725, Sunshine, VIC 3020, or Fax to (03) 8360 8699.
  3. Enter vaccination details directly via the web by logging on to the HPV Register secure web site. To enter vaccination details using the secure web site you must be a registered on-line user. GPs can request on line access by completing an Application for On-Line Access Form and faxing it to the HPV Register at (03) 83608699. 

Qld, WA and NT general practitioners

Please send HPV vaccination dose information directly to your Health Department. There is no need to notify the HPV Register of HPV vaccinations if these have been sent to your state health department, as Qld, WA and NT Health will submit HPV vaccination data to the HPV Register on your behalf.

School and Community providers except in Qld, WA and NT

  1. Load a file of vaccination episodes extracted from your vaccination management software such as ImPs or Vacciwise directly via the web by logging on to the HPV Register secure web site. To enter vaccination details using the secure web site you must be a registered on-line user. School and Community providers can request on line access by completing an Application for On-Line Access Form and faxing it to the HPV Register at (03) 83608699.
  2. Enter vaccination details directly via the web by logging on to the HPV Register secure web site. To enter vaccination details using the secure web site you must be a registered on-line user.
